Table openmrs.xforms_xforms doesn't exist

I am trying to access the patient data from the Find/Create Patient Tab. When I search for a patient, let’s say John and I click on it, I am supposed to be redirected to a page with patient details (contact details, visits, vitals etc). Instead I get Server Error and I have no idea why. My Tomcat log, keeps showing this error

ERROR - JDBCExceptionReporter.logExceptions<234> :2015-11-26 09:33:33: Table ‘openmrs.xforms_xforms’ doesn’t exist Any idea what might be wrong here? Any help would be greatly appreciated! Thank you!

Try uninstall the xforms module and test again.

I have the same issue. Its installed and i have tried to install and uninstall it again but it do not works. I have posted a detailed issue here OpenMRS Form Entry Module
