XForms Error new - Pastebin.com

This is a new error in a succession of errors re Xforms. Slowly but surely troubleshooting. Forms seem to work now as I can add concepts etc, but the Xforms integration giving above error.

I am also unable to get the forms at the patient encounter when trying to enter Forms. Something about Null-bind for the form.

Vitals form seems to work.

Which browser are you using? Does it still happen when you use Firefox?
