Remove hard-coded concept ids from OpenMRS constants

Hallo guys

I am doing my first ticket https://issues.openmrs.org/browse/TRUNK-4681 The idea is to have configured concept ides for the OpenMRS rather than depending on reference values that are generated after a period of using OpenMRS on a device. Which may not be appropriate for first time users. At least that’s how i have understood the ticket.

I would like to get some suggestions on how to make this happen
