Problem with adding an Observation in OpenMRS

Hello, I have got a problem when adding an Observation in the graphical user interface of OpenMRS. Always an error code like: TypeError: response.data.results[0] is undefined, and also an error code about the FullySpecifiedName in my concept occurs Even though I already createt an Observation template for a patient and published it within my system but when I want to add it to my patient I can not find it.

Any suggestions about this problem?

How are you trying to add obs, can you share the server logs and we check in or screen shot
