Observation form disable duplicate entry in the same day

I’ve setup few observation forms and one of them is “Immunization”

I’ve allow registration of more than one instance for a patient. But right now patient can register for the same immunization type in the same day. So i want to validate not more than the same immunization can be provided on the same date. How do i achieve this?
