Manage Report in administration

Hi, i need your help when i try to define a new report in cohort querry it displays this error. Your help will be aprreciate.

What are the exact steps to reproduce on that server?

in the Administration on the section Reports ,select Manage reports and click on " Create Cohort Report"

sorry for the delay, still giving me the same error when i click “save” .

I was not clear the error comes after i filled all the details to create cohort Report and i click on “save”.
