Making Patient history with coded answers

Hello everyone, I am new to OpenMRS and I am trying to create a patient history list where it will have questions and coded answeres like this:

Diabetes yes [] no [] Hypertension yes[] no[] Eye problem no [] cataract [] retinopathy []

For the diabetes I found a conceptID that was coded, 119481.

For hypertension I could not find a similar conceptID. Do I need to make my own or can I use a concept ID and do something to make it work?

I don’t think you want to record a patient history this way in the database. You can provide a list of likely history entries, but I think you want to store them in an array. Do you have CIEL loaded?
