Html Form Not show in Reference Application interface

**Reference Apllication: 2.10:

Hi OpenMRS community, i just installed OpenMRS and all Reference Application Addons on my Windows 10 machine using Xampp. I tried also to create a custom form through the Html Form Module but unable to use it for a patient i already created. The form is only shown whe i use legacy ui interface but not when am using new Reference application Interface. Any guidance please. Thank
