How to set multiple choice forms (ampathforms) to source data from concepts?

Hi all

How to make a multiselect form (ampathforms), a control to search for values from the diagnosis concept reference book ‘icd-9-cm-diagnoses.csv’, using the ICD-9-CM data source (concept_reference_source), but using the Diagnosis concept class? There are examples of data sources in the ‘diagnosis_core.csv’ concept, but I don’t understand how to build a path - a filter for my task. (Same as mappings)

Can you talk more about why you are looking to use ICD-9-CM codes for diagnoses and not using interface terms (like CIEL) or even a later version of ICD-10?
