Error created when Dr. writes diagnosis in openmrs

Hi,

We have been facing an issue offlate, where when the doctor provides a diagnosis, the error message are shown as:

  1. “If there are multiple reloads, please contact the system admin”
  2. “failed to create an encounter”.

If anybody has faced similar issues before, it would be great if they could provide corrective steps. Also, help us understand what is the root cause behind the same.
