Create a new field in patient registration

Hi in creating new patient I need to add a new field (gov medical ID), but in the setting/admin I can not find the form to add the field to. I want to add a field when a user is registering a new patient from http://xxx.xx.xx/openmrs/registrationapp/registerPatient.page?appId=referenceapplication.registrationapp.registerPatient
