Ability to add notes/capture reason while cancelling an appointment

Hello all,

As part of a client engagement, we have a requirement to add notes while cancelling an appointment in Bahmni. Below is a screenshot of the existing confirmation popup which appears while cancelling an appointment. We are thinking of having a text box to add in notes along with the confirmation. Further enhancements would include templatized cancelled reasons. Let me know if there are any feedback/requirements on the same.

I guess it would be good to provide an “optional” text box on the pop-up itself, to provide “reason”. And if there can be some buttons / dropdown, that can pre-fill some template strings (“Patient requested”, “Doctor not available”, etc), then people can quickly select that, and the comment box will get populated. Saves time.
