Yes that is exactly what i mean. The design surface does not seem to have all the radio buttons for visit type as i advised above.

You do not have to create a new concept. You can just ensure that the missing radio buttons are added.
So on the forms in the production environment users sometime wants the response to question changed. In those cases there are some of the response we remove them when designing the form. If that is causing the problem it means whenever we want to make those kind of changes we are going to have to create new concepts?
What about the textboxes missing they values what could cause that?

No, you do not have to create new concepts. This was for troubleshooting purposes, where i wanted to narrow down to the conditions which lead to this bug. That is why i asked you to create a ticket for it to get fixed.
Are you able to reproduce this too with a sample form?
If you remove any of the radio buttons, you should be able to reproduce it.
I was not able to reproduce the problem on the textboxes.
If I remove the radio button on the form I get the problem.

First of all thank you for the support in troubleshooting the problem this far. But I have encounter same problem on the other fields of the form one them has a dropdown list. The weird thing is that when you open the form to edit it the first time it doesn’t retrieve its value. When you fill in the missing value and submit the form, then edit it for the second/third time that is when it doesn’t lose its value. Here is the log file
