Lab order Empty , doesn't show required Data , required fields are empty , How to fill them ?

dear sir, first i tried delete it but it has foreign reference with some other test. plz check below screen. then i just make it retire .

while importing meta data again it shows me following exception cause. Caused by: org.openmrs.api.DuplicateConceptNameException: ‘CBC’ is a duplicate name in locale ‘en’ for the same concept at org.openmrs.validator.ConceptValidator.validate(ConceptValidator.java:194)
