Where to place the Human Genetics Unit Data Collection Form?

I am in the phase of solving some issues with the HTML form viewing. In order to do that properly I need to figure where are we planning to place the HTML form at. Are we gonna keep the form as a vital or as an encounter.

If the HTML form created as a vital the data would be displayed here

This is how i would look at it. Is this data considered to be part of vital signs? If yes, then you know. :smile:

I would create a new page just to test the HTML Forms, to ease the debugging.
