While unifying the Service Queues visit tabs onto the patient chart’s shared visit summary ([O3-5763](https://openmrs.atlassian.net/browse/O3-5763)), the queue’s inline visit-note widget was replaced by the chart’s read-only Notes tab. [Here](https://github.com/openmrs/openmrs-esm-patient-management/pull/2600#discussion_r3604418819), we agreed to keep the ability to _write_ a note, and it’s back as a **Visit note form** launcher but that widget predates two other note mechanisms, so I’d like input before settling on it.

The options:

- **Visit note** (`esm-patient-notes-app`) — encounter-backed, carries diagnoses, visit-scoped. A durable clinical record.
- **Patient sticky note** (`esm-patient-notes-app`) — a standalone obs, patient-scoped, one current note edited in place.
- **Ward patient notes** (`esm-ward-app`) — obs written via an encounter, visit-scoped, edited in place with history.

Today it’s the **visit note** , same as before this change — the button opens the chart’s visit note form, which writes an encounter of the visit-note type with the clinical note and diagnoses.

Which of the three should it be?

@veronica @fanderson thought you guys might have some opinions on this so pinging you on it 😃 @ujjawalprabhat I think you’d probably get more feedback raising this on the squad call as well

@ujjawalprabhat the Visit note makes sense to me.

The Ward patient notes is used typically for admitted patients.

I think the sticky note function is more for quick references to notes or reminders.
