How to customize the read-only view of this form? How to list obsgorup questions as list?

I tried the view icon and I saw the read-only view of data.

but my point is to see the obsgroup data in properly listed and easily readable manner like bellow screen-shot,

I can reproduce the same result in qa-refapp.openmrs.org if you could import CIEL Concept Dictionary.
