How to add a form label using configuration files?

How do I add a label (such as "DM patient’s goal 130/80"in the screenshot below) to a form using JSON configuration files?

Hi @dsurrao,

Please add description for the concept in OpenMRS as shown in below screenshot. This can’t be done in configuration JSON files.
