Error while edittng bahmni observation template.

We installed bahmni successfully. We are planning to use it for now to document our diabetes registry.while I was editing the intake form, I encountered an error below.please help how to trouble shoot it.

N.B. Though i am medical doctor and can not perform complicated steps. If you can suggest to me, I can ask IT guys to help me.

and can you drop detailed error log at hastebin.com ?
