Convert Html code to a custom module

Hello,

I have html code that I want to convert to a custom module that can be seen in the home screen when openmrs is run on a public server. The directions in the documentation were very confusing. Can someone guide me as to what modules I need to download and what version of openmrs I need? Also if they can guide me using a very simple html script the steps I need to follow I would really appreciate it

Yes I have. I am just not sure what versions of openmrs it is compatible with. Also, once I have created the form, I cant see it in my home screen.
